Terms and Conditions

These Terms and Conditions (“T&C”) govern the contractual relationship between the healthcare provider and the patient – the client
(healthcare recipient).
Healthcare provider: STOdent s.r.o., reg. No.: 03324575, hereinafter also referred to as “STOdent” or “Provider”. The terms “treatment”, “care” or “intervention” mean the provision of healthcare services as defined by law. The term “physician” means a healthcare provider acting for the Provider, both in the position of a dentist and in the position of a dental hygienist. The term “office” means an integrated operational unit providing healthcare, consisting mainly in a dentist chair with the corresponding technological, diagnostic and therapeutic equipment, including the physician, the assistant and ancillary healthcare staff.

Obligations of the Provider

STOdent undertakes to provide healthcare in the area of practical dentistry at the level of contemporary modern medical knowledge, in line with the best interests of the patient’s health condition, and in accordance with applicable legal regulations and internal regulations of the Provider. STOdent undertakes to keep the patient’s medical records as required by law. STOdent processes information stored in the documentation according to the personal data protection regulations and other legal regulations. Before any medical intervention, the patient will be informed by the physician, to a sufficient extent, about the patient’s oral condition, about the proposed treatment (therapeutic plan) and about the nature, consequences and possible risk associated with that medical intervention. In the case of existence of several variants of treatment, STOdent will acquire the patient with each variant and its cost. The Provider is obliged to schedule patients’ appointments for agreed times as to minimize the patients’ waiting time. The Provider is obliged to comply with all ethical principles of the physician/patient relationship, to minimize stress and potential pain of the care provided and to communicate with the patient to make sure the patient understands everything. The Provider is obliged to fairly communicate (before treatment) and subsequently bill (after treatment) the medical services provided. STOdent reserves the right to refuse or terminate treatment if:

  • the patient fails to comply with the proposed treatment procedure
  • the patient repeatedly fails to attend a medical appointment without a timely excuse
  • the patient fails to comply with the Provider’s internal regulations or restricts the rights of other patients
  • the patient’s requirements are inconsistent with the physician’s professional beliefs
  • if other statutory reasons apply (operational or capacity reasons)

Obligations of the patient

The patient is obliged to attend the appointments for agreed medical interventions in accordance with the treatment plan, preventive examinations and the plan of dental hygienist’s interventions. An appointment is a time and date entered in the Provider’s system and the Provider must communicate it to the patient orally, by telephone or by email. The Provider is entitled to change the date or time of an appointment and inform the patient (orally, by telephone or by email) about such change. The patient must be aware that in the case of his or her non-attendance, the Provider incurs damage proportional to the minute rate of the office, which the Provider is entitled to claim from the patient. In the case of patient’s non-attendance (as referred to in the previous paragraph), the parties hereby agree on a contractual penalty the Provider is entitled to claim from the patient. The contractual penalty is 75% (seventy-five per cent) of the multiple of the office’s minute rate and the agreed planned length of the appointment.  The contractual penalty is payable on the day of the non-attendance and the Provider is entitled to bill it to the patient subsequently. The patient acknowledges that the motivation of this provision is not to charge contractual penalties but to prevent non-attendance without excuse and downtimes of a costly medical facility.
The patient may excuse from an appointment in advance and schedule another appointment. In such a case, an excuse must be made no later than 24 hours in advance in a demonstrable manner – in person or by telephone at the STOdent reception, by electronic mail at zubar@recepcestodent.cz or a text message at +420 720 656 274.

The patient is obliged to:

  • comply with the proposed treatment procedure
  • comply with the Provider’s internal regulations
  • truthfully inform the physician about his or her health condition and related issues
  • take adequate care of his or her oral health as to avoid all avoidable (preventable) health risks
  • accept preventive examinations and preventive dental hygiene as an indispensable part of treatment
  • provide assistance during medical interventions and checks of the course of the treatment process
  • maintain a high standard of his or her dental hygiene

Premium treatment

STOdent offers a wide range of premium dentistry interventions, with a focus on quality, modern technologies and mini-invasive (painless) interventions. Also in the area of premium care there are always several variants of treatment and the patient can choose from several options. The patient is always informed about the possible variants of care and their prices before receiving treatment. Health insurance companies in the Czech Republic cover only a small part of the costs of high-quality dentistry and, moreover, only if inexpensive (obsolete) materials and procedures are used—these are not, however, used by our clinic. Most types of treatments that lead to a long-term maintenance or a high-quality replacement of your teeth are, unfortunately, currently not covered by health insurance companies or are covered only partly and patients must pay for the rest of the treatment provided. Treatment fully covered by health insurance companies: initial and preventive examinations, making of an X-ray image. Treatment partially covered by health insurance companies: prosthetic laboratory products – crowns, bridges, inlays, some removable tooth replacement products. Treatment not covered by health insurance companies: fillings, endodontic treatment, dental implants. Our goal is to provide high-quality treatment that leads to an aesthetically perfect and long-lasting healthy smile. Individual possible treatment options and a quotation are communicated to the patient by the physician upon examination, before the actual treatment. In the case of extensive (clinically- and price-intensive) reconstructive interventions, the physician will prepare a written price plan for the patient. In the case of complications and more time-consuming treatment, the planned price may only by increased in accordance with the current minute rate of the office. The office’s minute rate is available in the Provider’s price list. The purpose of dental hygiene is focus on early prevention. The minute rate represents all the costs of the intervention in the office and includes professional work, the technologies used, common (standard) materials, and ensuring the hygienic regime of the office. Special materials (disposable instruments, special masses, etc.), implants and prosthetic products are not included in the minute rate and are billed separately to the patient. Payment is always made based on a detailed specification for the client. Payment is made in cash or by card. In the case of external production of prosthetic products or purchase of specific implants, an advance payment of 50% may be requested.

Complaints

STOdent provides a 2-year guarantee (from the moment of completion) for all fillings and prosthetic work (crowns, bridges). The guarantee does not apply to temporary work, endodontic treatment, dental procedures of a service nature (dental hygiene, tooth whitening, interventions on soft tissues, etc.). The reasons for a complete or partial cancellation of the guarantee include: negligent oral hygiene, failure to comply with medical advice, improper use of dental prosthesis, untreated dysfunction of the temporomandibular joint, systemic diseases affecting the oral health condition (diabetes, epilepsy, osteoporosis, cytostatic therapy, etc.), and head or neck injuries with the possibility of tooth or jaw injury. A complaint must first be filed by the patient directly with the Provider, who must deal with the issue as part of the process of recording complaints and to handle the issue within the statutory time limit.

Final arrangements

The contract enters into force on the date of patient registration – his or her signature and expression of consent on the registration form. The text of the T&C is available to patients at www.zubartrutnov.cz and at the STOdent reception.
